The position includes a sign-on bonus and is based in the Youth and Families Division, with clinical duties in the Supervised Independent Living (SILP) for young adults in foster care in the Bronx, and two group residences: Kaplan House (Manhattan) and Cedar Knolls House. Position Overview

The Family Nurse Practitioner will evaluate clients for medical conditions, provide treatment according to established best practices, educate on healthy living and disease prevention guided by trauma-informed care that incorporates harm reduction and motivational interviewing, and collaborate with mental health clinicians to achieve optimal health outcomes. Key Responsibilities

Work scheduled hours at Kaplan House and Cedar Knolls House; SILP youth seen at one of these locations. Most sessions are in-person; some may be via telehealth. Provide comprehensive primary care services, including histories, physical exams, and diagnoses. Emphasize preventive care and medical screenings based on established guidance. Order and interpret diagnostic tests; use results to formulate treatment plans. Treat acute and chronic conditions with appropriate follow-up and monitoring. Refer to subspecialists when indicated and integrate recommendations into a comprehensive approach. Prescribe medications in accordance with best practices. Consider a range of treatment modalities and interventions to improve client conditions. Educate clients about their conditions and preventive/therapeutic measures. Utilize motivational interviewing techniques (e.g., SBIRT) for behavioral change. Stay up-to-date with innovations in primary care and nursing practice. Understand the interplay between mental and somatic health; monitor physical side effects of psychiatric medications. Communicate with interdisciplinary teams to ensure holistic understanding of clients’ conditions. Maintain medical records promptly and accurately using the organization’s Electronic Health Record. Rotate responsibilities for urgent after-hours issues. Initial admission assessment and annual re-assessments (more frequent as clinically indicated); provide medical health assessments and treatment plans for residents. Provide health education and counseling on health and sexual/reproductive health. Participate in practice improvement projects as initiated by program/Medical Standards and Quality Improvement. Monitor client progress; recommend discharge and aftercare planning medical needs. Communicate youth needs to program staff and interface with the clinic director as needed. Other duties as assigned. Education/Training Required

Licensed and registered as a Professional Nurse (RN) in New York State. Certification as a Nurse Practitioner by the New York State Education Department (NYSED). Experience/Language

Must have more than 3,600 hours of clinical experience as a Family Nurse Practitioner; no physician collaboration required. Community settings experience preferred but not required. Spanish, Russian, or Creole/French language skills preferred but not required. Computer Skills

Working knowledge of Microsoft Office, Outlook, and Word. Ability to learn systems such as MyAvatar, Connections, and Tableau. Visual and Manual Dexterity

Ability to read paper and electronic documents and perform data entry; adequate manual dexterity for office tasks. Work Environment/Physical Effort

Ability to lift up to 25 lbs; Cedar Knolls House and Kaplan House are walk-ups without elevators. Office-like noise levels typical of health-related environments. Benefits
