Part Time/Weekend Physical Therapist Case Manager Coverage Area:

Folsom In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment. What you’ll do

As a Physical Therapist Case Manager, you will: Perform an in-home evaluation and develop a care plan; coordinate with PTAs to follow the plan. Help patients with mobility, movement, and regaining independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure quality care. Evaluate and report patients’ progress to the MD. Why you should come work for us

Current unrestricted CA Physical Therapist License Current BLS/CPR Certification Valid CA Driver's License Knowledge of local community resources Knowledge of development, behavior related to illness/injury, and cultural influences on health care Knowledge of state and federal homecare regulatory guidelines preferred Home health care experience highly preferred Experience with high acuity patients is a plus Experience working with an interdisciplinary team is desired Case Management fundamentals are desired Self-directed, empathetic, critical thinker with outstanding interpersonal and leadership skills Organized Compensation : $120,000 - $135,000 a year Wage range for Physical Therapist, Level 1: 120,000 - 135,000 plus a 10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience. 21st Century offers generous bonus plans, comprehensive health benefits, and 401k (up to 4% match). As a practice, most PT Case Managers are hired as PTIs. When promoted to PTII (supervising 2 PTAs, handling a larger caseload, and higher autonomy), minimum salary is 135,000 plus 20K bonus (paid quarterly).
