About the Role:
This is a mobile, high-touch position. You’ll meet patients in homes, hospitals, or senior living settings to assess eligibility and educate families about hospice services.
Day-to-Day Includes:
Completing patient assessments and initiating care
Offering families a steady hand during emotional decisions
Communicating with physicians and team members
Accurate charting and efficient case handoff
What You Bring:
Licensed RN in MN
Experience in hospice, oncology, or end-of-life care
Ability to connect and comfort
Skilled in time management and prioritization
Dependable transportation and driver’s license
