Job Description Summary: Job Description: Part-time opportunity: Tuesday & Friday with one Saturday per month. The Primary Care Nurse (PCN) provides high-quality, comprehensive, professional nursing primary care to individuals and families across the lifespan. As part of a clinical team, the PCN works at the top of licensure to enhance safety, effectiveness, and efficiency of care through professional nursing responsibilities and delegation of technical and clerical functions as appropriate to medical assistants. The PCN offers health education on preventive, chronic, and episodic health issues. They collaborate with primary care providers to coordinate care, support transitions, and counsel, educate, and support patients in improving health outcomes. The PCN applies principles of advanced access, planned care, the chronic care model, and rapid cycle performance improvement using the PDSA approach at CHCI. Responsibilities: Support primary care of patients under the planned care model. Collaborate with PCP to identify needs for preventive care, evidence-based chronic disease management, and care coordination. Provide health education. Work with patients to identify and monitor self-management goals. Collaborate with community-based providers. Follow up on emergency room visits and hospital discharges. Assist providers with exams, treatments, and procedures as needed. Ensure safe, accurate, and appropriate collection, storage, and processing of biologic specimens. Administer medications, vaccines, and immunizations as ordered, adhering to protocols. Monitor patient responses and report adverse reactions. Educate patients on medications. Perform nursing activities such as wound care, catheterization, etc., after initial prescription. Document all patient care accurately and timely. Schedule tests and referrals. Maintain infection control standards according to protocols and OSHA/CLIA standards. Serve as a resource for medical assistants regarding patient preparation and procedures. Monitor and follow up on abnormal test results such as Pap smears, STDs, lead screens, etc. Oversee supplies and equipment maintenance, ordering as necessary. Ensure compliance with mandatory training requirements. Use electronic health records effectively for timely and confidential patient information management. Education:
