The Community Health Worker (CHW) is a trusted member of the community who supports individuals and families by addressing health-related social needs such as housing, food security, transportation, and access to care. This role bridges the gap between healthcare providers, social services, and the community to improve health outcomes. The CHW helps community members access needed health and social services, make meaningful connections with community partners, and is an advocate to address systemic identified barriers to care. A typical day could entail meeting with patients at One Community Health or in the community and coordinating with community partners to connect clients to needed services. *This is a grant funded position for a two-year term* Minimum Qualifications Knowledge, Skills, and Abilities Knowledge and understanding of the communities in Klickitat and Skamania Counties. Provides culturally and linguistically sensitive care coordination, using a client- and family- centered approach. Demonstrates professional and effective communication skills, including written, verbal and nonverbal. Proficient in Microsoft Office applications and related software. Able to appropriately document and communicate using the electronic health record (i.e., Epic). Maintains a positive attitude and provides respectful, professional customer service. Adheres to HIPAA regulations and respect clients’ privacy rights. Exhibits a high level of emotional intelligence, ensuring patients and team members feel heard, respected, and valued. Demonstrates a commitment to ongoing professional development and continuous learning. Ability to travel within the Klickitat and Skamania Counties; has a valid driver’s license and insured vehicle. Preferred: Expectation to excel and be part of a high functioning team. Understand and work within a systems perspective. Education High School graduate or equivalent Current CPR/BLS Certification in accordance with OCH Policy Successful completion of credentialing and privileging in accordance with OCH policy BLS/CPR Certification (must completed within 30 days of hire) Preferred: Some advanced education such as graduate of an accredited medical assisting program. Successful completion of a Community Health Worker formal training program and Oregon State certification is preferred. Otherwise, a commitment to completion of such program within 1 year is required. Experience 2 years’ experience in a health related or social service-related field A combination of equivalent education and work experience. Preferred: Certified Community Health Worker Experience working with low-income populations and their families. Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish. Milage Reimbursement: In accordance with OCH policy.
