Position Description Mental Health Resource Center is looking for a Psychiatric Nurse Practitioner to join its Florida Assertive Community Treatment (FACT) Program that serves individuals in the Tampa, Florida area.
The FACT program serves adults with severe and persistent mental illness, who may also have a co-occurring substance abuse disorder. The program’s multidisciplinary staff obtain or directly provide all mental health services to participants, primarily in their homes or the community, including: Psychiatric Care, Medical Referral and Follow-Up, Individual Supportive Therapy, Crisis Assessment and Intervention, Substance Abuse Services, Employment Support Services, Support in Activities of Daily Living, Social Network Support and Development, Peer Recovery Support, and Case Management Services.
A minimum of 75% of the FACT team’s services are provided in the person’s home or community environment. Company vehicles are available; however staff may also need to use their personal vehicles to provide services.
The essential functions of the position include, but are not limited to:
Essential Functions / Responsibilities
Conducts psychiatric assessments including psychiatric history, course of illness, response to treatment, mental status examination, and DSM-V/ICD-10 diagnoses; assists the team in determining the best treatment approach, with input from the individual and the family.
Provides medication management and oversees the medication self-administration program for all persons served by the FACT program to ensure symptom reduction and stability; works closely with the FACT team and utilizes treatment strategies which include: assessing each person’s behaviors and symptoms of mental illness and prescribing medication in response to the symptoms; regularly reviewing and documenting the person’s symptoms and response to treatment; educating the person regarding symptoms and the effects/side effects of medications; assisting the person to identify symptom patterns and develop internal/behavioral/adaptive methods to lessen effects; monitoring, treating, and documenting side effects; maintaining a positive, therapeutic relationship; using new atypical medications with greater effectiveness and a better side effect profile; and simplifying the medication regimen.
Completes screenings, assessments, and/or regular visits in both office and field-based settings as identified on the treatment plan, including visits to hospitals, CSUs, jails, or other community living environments.
Attends and participates in the team’s daily organizational meetings to assess patient status and progress, coordinate treatment activities, and develop treatment solutions.
Coordinates, with the nurses, the medical treatment of all individuals enrolled in the program and consults with staff and other agencies regarding diagnostic, medical, and treatment problems.
Ensures compliance with State Regulations, MHRC Policies, Procedures and Standards, and the Program for Assertive Community Treatment (PACT) manual.
Position Requirements
Master’s Degree in Nursing and current APRN Florida State Nursing license required.
ANCC certification in Psychiatric Nursing required.
DEA Certification required.
One year experience in psychiatric nursing with individuals with severe and persistent mental illness required.
Proficiency with MHRC Electronic Health Records (EHR) and Patient Information System within three months of employment.
Strong communication skills and ability to interact with internal and external customers, including patients, families, caregivers, community service providers, supervisory staff and other department professionals.
Commitment to complete and confidential documentation in compliance with internal and external guidelines for content, accuracy, and timeliness.
Position Details This is a full-time 32 hours per week position.
