Shift: M-F 8-5 Job ID: 418044 We are seeking a Diabetes Educator who thrives in a fast-paced environment. This role involves assessing, goal setting, educating, and providing behavioral interventions and ongoing case management to patients and caregivers to promote diabetes self-management in both inpatient and outpatient settings. Qualifications: Bachelor's degree required; master's preferred. CDCES from the Certification Board of Diabetes Care and Education OR BC-ADM from the Association of Diabetes Care and Education Specialists required. At least 1 year of experience in specialized diabetes care as a licensed dietitian or nurse. All successful candidates will be required to undergo respiratory fit testing to ensure workplace safety, in accordance with occupational health standards. Responsibilities: Teach patients and families skills for diabetes self-management, assessing their knowledge and tailoring education plans based on social, economic, cultural, and age-related needs. Evaluate patient needs periodically and suggest appropriate management strategies, including insulin injection management (IIM) and diabetes technology. Document all patient care activities accurately within medical records. Develop and distribute educational materials for staff, patients, caregivers, and the community. Maintain current knowledge through continuing education and staying updated with evidence-based practices. Implement protocols for diabetes management. Provide case management services, including coordination of care, requests for medical equipment, and preauthorization processes. When appropriate, assess and provide nutritional care, especially if holding an RD/RDN credential.
