The Clover Care Services organization delivers proactive support and care to our members through our Clover Home Care teams and provides quality improvement services to our aligned providers through our practice engagement team. Clover has built one of the most proactive, data-driven healthcare service platforms and is excited about how technology enhances our ability to drive transformative results for both patients and providers.
We are seeking a Nurse Practitioner (NP) to conduct comprehensive health assessments (Clover Care Visits) for enrolled Medicare Advantage members. You will also perform follow-up visits for members recently discharged from a hospital or post-acute care facility, as well as high-need patients. Visit assignments will be based on staff availability and member needs.
We are seeking full-time Nurse Practitioners to serve Hudson County! This is a full-time position with a Monday-Friday schedule.
Responsibilities
Deliver preventive care to Clover members in their homes using Clover Assistant and EMR.
Conduct comprehensive health assessments, evaluating medical history, physical health, environment, and psychosocial status.
Demonstrate clinical competency in chronic disease management within your scope of practice.
Document progress notes and prepare member workups and encounter summaries using Clover Assistant and EMR.
Assist at-risk members in transitioning from hospitals and post-acute facilities to home.
Create personalized care plans in coordination with members’ primary care physicians.
Actively close care gaps, which may include ordering labs, writing prescriptions (in collaboration with the Primary Care Provider or supervising physician), and coordinating follow-ups.
Perform point-of-care tests such as A1c, spirometry, and other diagnostics.
Support Clover Care Visit program goals, ensuring accurate documentation, high performance on federal quality measures, and successful referrals to network providers and internal clinical teams.
Coordinate and communicate with members’ interdisciplinary care teams.
Participate in ongoing training and meetings, either onsite or remotely.
Qualifications
Are based in Hudson County, New Jersey (required).
Are a certified Advanced Nurse Practitioner in a specialty area (required).
Hold a current, unencumbered NJ RN license.
Have an active DEA and CDS registration.
Have at least one year of experience as an Advanced Nurse Practitioner.
Earned a Bachelor’s degree (BSN) from an accredited nursing school and a Master’s degree in advanced clinical practice nursing or completed a post-Master’s certificate program for Nurse Practitioners.
Have a current Basic Life Support (BLS) provider card.
Have home care experience (preferred).
Are willing to travel to members’ homes and have reliable transportation.

A reasonable estimate of the base salary range for this role is $140,000 to $165,000. Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, certifications, etc.
