Overview
JOB PURPOSE: Performs duties of distribution, documentation and compounding related to drugs, solutions and equipment to meet the needs of patients. Responsible for the reviewing, compounding, dispensing and monitoring of medication therapy for home infusion and specialty pharmacy patients which includes dosing variations, nutritional needs and disease states associated with the various age groups. Communicates with other medical personnel involved in the care of the home patient. Works with billing and intake staff to ensure quality care coordination and proper billing guidelines are followed. Responsibilities
Distribute, document and compound medications and related solutions and equipment for patient needs. Review, compound, dispense and monitor medication therapy for home infusion and specialty pharmacy patients, including dosing variations and disease states across age groups. Supervise pharmacy technician activities, including monitoring orders filled by auxiliary personnel. Maintain competency and professional practice in accordance with current USP 797/800 rules affecting home infusion pharmacy. Provide patient-focused clinical activities through regular phone contact to ensure medication adherence and optimal therapy outcomes, in alignment with Joint Commission Standards for Homecare. Communicate with other medical personnel involved in the home patient9s care. Collaborate with billing and intake staff to ensure quality care coordination and proper billing guidelines. Qualifications
MINIMUM EDUCATION REQUIRED:
Graduate of an ACPE accredited school of pharmacy with a Bachelor00s Degree or higher in Pharmacy. MINIMUM EXPERIENCE REQUIRED:
NA MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Licensed as a registered pharmacist in the state of Georgia. ADDITIONAL PREFERRED QUALIFICATIONS:
One year experience in acute care or Home Infusion preferred.
