JR55179* Full-Time position* BenefitedConnected Home Health & Hospice is looking for a Physical Therapist who is a leader in their profession, an effective communicator, will embrace our core values, and who will share our passion and energy.

Our Physical Therapists are supported by a trained Physical Therapy Assistant.

Team meetings can be as easy as a phone call!**About the Opportunity*** Provides physical therapy services to patients in their homes according to a written plan of care* Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences* Participates in discharge planning for patient* Works closely with interdisciplinary team to ensure appropriate care coordination* Prepares and submits a clinical progress or discharge summary based on the attainment of goals as directed by Connected Home Health policy* Supervises Physical Therapy Assistants according to agency policy and state regulations**Requirements*** Degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization* Licensed to practice as a physical therapist within the State of Oregon* Two years of appropriate experience as a start of care physical therapist.

Community/Home Health experience is preferred.* Demonstrates good verbal and written communication and organization skills* Current BLS for Healthcare Workers from the American Heart Association* Must be a licensed driver with an automobile that is insured in accordance with state or Agency requirements and is in good working order**About the Company**Connected Home Health & Hospice is a Medicare certified Home Health Agency providing skilled nursing and therapy services consistent with our mission to provide life changing service.
