Primary Care Physician (PCP) The Primary Care Physician in our organization demonstrates accountability for outcomes, coaching for health, and simplifying for action. The PCP has a panel of up to 450 patients, acts as a health coach, and simplifies behavior change for patients and care teams.
Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors.
Coaching for health: The PCP acts as a health coach, rather than just a consultant for sickness, by helping patients set short and long‑term health goals, partners with the patient to work toward the goals, and frequently follows up on those goals on the path to improved health for their patients.
Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient in helping them achieve their goals, and when leading their care teams toward their performance goals.
We are an outcomes‑focused, value‑based organization. For each PCP’s panel, metrics such as patient admissions per thousand, appointment slots per day, CGCAHPS (patient experience), clinical gaps closures, and medical cost measures are regularly measured. Each PCP will have goals for these metrics and will be expected to work toward those targets with their center and market leadership as well as their care teams. Culture is very important; PCPs are leaders and are expected to champion a positive culture of love, accountability, and passion along with center leadership.
Essential Job Duties / Responsibilities
The PCP independently provides care for patients with acute and chronic illnesses encountered in older adults.
The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, while remaining involved and responsible for detailed care.
It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
The PCP leads their care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
For patients unable to come to the office—in hospital, SNF, LTC or homebound—the PCP will engage the transitional care team and others including case managers, acute and transitional‑care physicians, and other resources that may be available depending on the market.
PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.
Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company.
Performs other duties as assigned and modified at manager’s discretion.
Knowledge, Skills & Abilities
Availability and accessibility for patients to build trust; PCPs are expected to be available for walk‑in visits and to answer phone calls and messages promptly.
Service orientation – provide care that they would want for a family member or themselves to each patient at every interaction.
Evidence‑based medicine – remains updated on evidence‑based medicine and recognizes lifestyle and nutrition impacts; stays current on clinical, nutritional, and lifestyle‑based interventions.
Physician leadership – willing to continuously develop leadership skills for benefit of patients, team, center, and company.
Quality – pursue high quality outcomes and quality measures, and continuously improve practice and operations.
Influence – competently and compassionately influence patients, teams, and themselves.
Self‑care – maintain well‑being physically, mentally, socially, and spiritually for physician wellness.
Proficient in Microsoft Office Suite and related productivity software.
Ability and willingness to travel locally as needed, nationally for initial training (2 weeks) and occasionally regionally and nationally for recruiting or training.
Fluency in English, verbal and written; other language fluency may be required in some centers.
This job requires use and exercise of independent judgment.
Education And Experience Criteria
MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
Must obtain or have a current, active State Medical License for the state(s) in which they will be working.
Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty preferred; board eligibility required.
Once board certified, maintain board certification by completing MOC, CME and/or retaking board exams as required.
Must have a current DEA number for schedule II‑V controlled substances.
Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days of employment.
