RN Surgery Nurse Navigator
Location: Syracuse Overview
The RN Navigator is a Registered Nurse whose clinical nursing experience guides patients and their families to informed decision making. Responsible for planning, guiding and evaluating the delivery of patient care; monitors patient progress, intervenes as needed to ensure compliance and optimum care, and collaborates within a multidisciplinary team to facilitate screening, diagnosis, therapeutic intervention, treatment and supportive care across the health care continuum. The RN Navigator coordinates all resources necessary to provide care and uses advanced expertise to identify, implement and improve processes to provide an integrative model of care and high patient satisfaction. The RN Navigator provides patient education regarding disease process and treatment throughout the health care continuum and promotes professional development while maintaining a professional, safe, and therapeutic environment for rendering care. Responsibilities
Guide patients and families to informed decision making and coordinate the delivery of patient care across the health care continuum Monitor patient progress, intervene to ensure compliance and optimal care Collaborate with multidisciplinary teams to facilitate screening, diagnosis, therapeutic intervention, treatment, and supportive care Coordinate resources necessary to provide care and improve care processes Provide patient education regarding disease process and treatment throughout care continuum Maintain a professional, safe, and therapeutic environment for care delivery Qualifications
NYS licensed/registered or eligible RN required Bachelors degree in Nursing or healthcare related field OR an AAS in Nursing with five (5) years post-licensure RN experience (preferably in specialty) Excellent written and verbal communication and interpersonal skills Current BLS certification through the American Heart Association or the American Red Cross by time of appointment Preferred Qualifications
Three (3) years as a Certified Nurse/Patient Navigator in the specialty area (preferred) Work Details
