Job Summary Under the supervision of the Clinical Supervisor for the Citywide Supportive Housing Program, the Clinical Social Work I/II (CSW I/II) works with San Franciscans living in Permanent Supportive Housing who struggle with issues of mental, physical health, substance use, and housing retention. The CSW I/II collaborates with community providers, links residents to appropriate outpatient services, and provides housing stabilization to prevent homelessness.
Responsibilities
Carry a caseload of residents from DISH supportive housing sites (Camelot, Le Nain, Empress, Windsor, PBI, Star), providing individual treatment including case management, psychotherapy, crisis intervention, and 5150 assessments.
Deliver substance use treatment, psychoeducation, group treatment, and community events; conduct intake, assessment, service planning, and consultation.
Maintain client records in accordance with California, City, and County requirements; participate in interdisciplinary treatment teams and attend weekly staff meetings and required training sessions.
Coordinate closely with property‑management staff from DISH and DPH Nursing, and with other service providers to develop treatment strategies, advocate for clients, and ensure comprehensive care.
Track client contacts in Epic and One Systems, complete initial and annual assessments, service plans, referrals, and linkages; coordinate with hospitals and clinic medical providers.
Perform outreach to clients in their homes, lift 50 pounds, walk, and climb stairs as part of daily duties.
Demonstrate commitment to anti‑racism, social justice, and respecting a diverse community of clients and peers.
Show understanding of mental health issues and sensitivity toward mental health clients, and work cooperatively in a high‑stress, high‑volume environment.
Requirements
CSW I: Master’s degree in social work, psychology, or related field from an accredited university and registration as an Associate Clinical Social Worker (ASW) or Intern Marriage Family Therapist (IMFT).
CSW II: Master’s degree in social work, psychology, or related field from an accredited university and possession of the requirements necessary for licensure as determined by the California Board of Behavioral Science.
Ability to work with clients and staff from diverse cultural and social backgrounds.
Excellent communication, interpersonal skills, and ability to follow oral and written instructions accurately.
Knowledge of medical and/or psychiatric terminology.
Ability to lift 50 lbs, climb stairs, and keep detailed, accurate records.
Demonstrated excellent attendance and reliability.
Background in case management.
Preferred Qualifications
Two years’ experience working with severely mentally ill adults, the homeless, and dual‑diagnosed populations.
One year of experience in community‑based case management and performing outreach to SRO hotels and scattered site housing locations.
