LPN - Clinic II (Meds)

Job Summary Under direction of a Registered Nurse and under the medical orders of a Practitioner, provides direct and indirect nursing care in an outpatient setting.
Responsibilities
Reviews and prepares medical records for assigned patients; initiates follow‑up for medical records as requested; reviews patient needs; identifies and prepares orders for patient needs for authorization by the practitioner and may accept verbal orders per policy.
Implements basic nursing procedures per plan of care; administers treatments, immunizations, and medications as prescribed by the practitioner; prepares medications for practitioner use; performs point of care testing as authorized; provides laboratory, x‑ray, and other reports to practitioner for review; notifies patients of normal results as directed by practitioner.
Documents implementation of orders/interventions; records findings, instructions, and patient responses on appropriate forms or online records using department and/or JCAHO standards; observes patient/family responses to plan of care; reports unexpected responses to RN or practitioner; collaborates with RN/Practitioner to revise care plan.
Performs efficiently in emergency patient care/clinic situations; follows established procedures; informs appropriate practitioner/RN; assigns basic nursing tasks to staff as appropriate and according to department/unit standards; provides clear directions; acts as resource to staff.
Follows the discharge plan developed by Practitioner or RN; processes and/or follows up with referrals as directed; provides instruction and pre‑printed information to patients; observes and documents patient/family response to instructions and reports any concerns.
Manages all patient vaccine data, including updating Hawaii Immunization Registry System (HIR) and Electronic Health Records; prepares immunization information prior to patient visit and administers immunizations according to health plan and VFC guidelines.
Identifies patient immunization requirements per CDC age‑specific guidelines; fills out appropriate paperwork for recording; facilitates vaccine inventory control at the clinic; orders vaccines and maintains inventory.
Maintains file records of vaccine arrivals in the I lot manager system; ensures immunization clinics comply with health plan, federal, and state guidelines; completes required VFC enrollment training and maintains all VFC program records.
Trains new and existing LPNs on emergency vaccine storage procedures; coordinates with state to maintain VFC inventory; generates reports on patient vaccination status and follows up for compliance.
Assists in ensuring compliance with infection control policies and medication administration, storage procedures, and health plan/COVID guidelines; provides educational materials to patients and collaborates with RN/Practitioner to revise teaching plans.
Participates in orientation of new LPNs; demonstrates proficiency in clean/sterile technique and clinical procedures within scope; follows high‑level disinfection committee guidelines for instrument handling.
Maintains, orders, organizes, rotates, and stocks supplies; schedules patients for routine and acute care and addresses care gaps; prepares and processes patient forms.
OHS only: Certification in Spirometry, Hearing Conservation, and Substance Abuse Specimen Collection or acquire certification within one year of hire.
Demonstrates knowledge, skills, and abilities to provide age‑appropriate and culturally sensitive care.
Performs other duties and accepts responsibility as assigned within scope.
Basic Qualifications
Licensed Practical Nurse (Hawaii) required at hire.
Basic Life Support from the American Heart Association.
Education
Hawaii State Board of Nursing approved curriculum for LPN licensure.
Additional Requirements
Demonstrated knowledge and skill in conflict resolution, customer service, interpersonal relations, oral communication, problem solving, teamwork, and written communication.
Experience talking to co‑workers, customers, outside vendors, and on the telephone.
Reading, writing, speaking and understanding English.
Training, giving, and receiving instructions.
Mathematical ability, attention to detail, concentration and alertness.
Preferred Qualifications
Previous nursing experience.
Previous clinic experience.
Proficiency in word processing and software.
