The specialty APP in the ambulatory care setting works in collaboration with a multidisciplinary health care team. The specialty APP is generally service/physician aligned, managing a distinct patient population in the outpatient setting. The APP functioning primarily in a clinic setting may be required to perform some coordination of inpatient care. The specialty clinic APP has a strong foundation in evidenced based practice/education and is responsible for ensuring adherence to clinical pathways/protocols and ensures an environment of safe quality care. The APP demonstrates an advanced level of medical knowledge, clinical/technical competence, sound clinical judgment, professional communication skills, timely and compliant documentation, and is responsible for professional development and competency validation. Position Shift:

Monday-Friday typical clinic hours. No call, no weekends, no holidays. 100% outpatient setting. Department Summary

The General Neurology Division oversees a wide array of neurologic conditions and is often the first point of contact for some of the most unique and complicated cases. Vanderbilt treats a high volume of stroke patients and offers access to testing, imaging facilities, clinical trials, and collaboration across subspecialties. Vanderbilt is an Advanced Certification Comprehensive Stroke Center located in the stroke belt region. As one of the largest stroke facilities in the Southeast, Vanderbilt treats over 1,000 acute stroke victims each year. Stroke care at Vanderbilt includes prevention education, follow-up care and rehabilitation through collaboration with physicians and researchers. Responsibilities

Assessment of health status; complete history and physical examination for acute and complex chronically ill patients. Order and collect data using appropriate assessment techniques and diagnostic procedures where indicated. Serve as first responder for nursing and house staff in patient status changes and in situations requiring procedural and resuscitative interventions. Assess risks associated with care of acute and complex chronically ill patients, including medication side effects, immobility, nutrition, fluids/electrolytes, immunocompetence, and invasive procedures. Coordinate daily clinic schedule with attending physician team; present patient report; collaborate in plan of care; follow assigned patient population. Collaborate with interdisciplinary team to diagnose acute and complex chronic conditions; manage diagnostic tests; interpret and supervise results; formulate differential diagnoses by priority. Develop and document plan of care with expected outcomes; apply evidenced based guidelines; implement and modify plan of care. Prescribe diagnostic strategies and therapeutic interventions (pharmacologic, non-pharmacologic, and surgical) as needed; perform advanced procedures consistent with privileges and competency validation (e.g., minor suturing, wound management, line/tube placements and removals). Maintain ongoing communication with the care team; promote continuity of care across outpatient care continuum. Document key components of patient progress in the medical record; ensure timely documentation meets compliance standards. Qualifications

Certifications:

Licensed Nurse Practitioner - Tennessee (LIC). Work Experience:

Relevant work experience; less than 1 year preferred. Education:

Master's degree. Preferred Qualifications: Previous experience in a high-volume clinic. Neurology experience. Outpatient experience.
