Job Summary And Qualifications The Case Manager RN Wound Care ensures high‑quality, patient‑centered care by managing rehabilitative care to ensure optimum outcomes. The CM provides, coordinates, and directs care specific to the needs of each rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions. The CM coordinates efforts within the Rehabilitation team. This is a hands‑on position. The nurse will primarily lead the inpatient program and round on inpatient wound care consults, assist and determine appropriate dressings, educate nurses on the inpatient units, etc. The role will also include outpatient clinic responsibilities.
Responsibilities
Prepare the Daily Wound Care Report: fax a copy to each inpatient unit and email it to the inpatient unit leadership group. Log in on iMobile and stay logged in until you leave for the day to allow communication regarding wound patients throughout the day.
Restock Wound Care cart.
Round daily with provider: message the patient’s nurse prior to seeing the patient. Let them know to pre‑medicate the patient if needed and encourage them to attend while wound care is seeing the patient to visualize wounds and dressing change instructions.
Picture and measure wounds.
Measure circumference as applicable.
Leave our contact card with patient.
Note on Wound Care Report: dressing change instructions, date seen by provider, and any other pertinent info such as discharge date and destination. Ensure outpatient referral is entered if applicable.
Check‑in on patients not on the provider’s list for that day: ensure dressings have been changed by nursing. Check in with the patient's nurse to see if they have any questions and ensure they understand dressing orders. See if the patient knows or is there notes by hospitalist or Case Manager regarding when the patient will be discharged and where.
Call OP AWCC and get the outpatient follow‑up appointment scheduled while at the patient’s bedside prior to discharge when possible.
Leave the patient our contact info card with their appointment information written on it and provide directions if needed.
Give patient/family an opportunity to ask questions.
Providers are to re‑evaluate patients every 7 days unless requested to re‑evaluate sooner.
When there is no provider on Friday, round on patients on Monday and make any needed changes to the plan of care.
On days with lighter census: utilize the daily wound care report and spot‑check patients who don’t have a wound consult entered but have potentially applicable wounds. Communicate findings and needs to the hospitalist.
Wipe down inpatient wound cart before returning to storage area.
Make a list of items needed to restock the cart for the next day.
Provide Manager Daily: total number of inpatient contacts, names of patients discharged with date and destination at discharge. If Case Management has set up home health upon discharge, please note the name of the company and names of patients/date/time with OP AWCC appointment scheduled.
Qualifications
Basic Cardiac Life Support must be obtained within 30 days of employment start date.
Registered Nurse.
Associate Degree, Bachelor’s preferred.
2+ years experience in case management OR 3+ years experience in a clinical setting.
WCON (wound care and ostomy nurse certification) preferred upon hire. If the candidate does not have it, they must have previous wound care experience and be willing to obtain this certification within 6 months of hire.
