We are seeking a skilled and compassionate Nurse Practitioner or Physician Assistant to join our growing team in Brooklyn, NY. Position Details
Schedule: Monday–Friday
No nights, no weekends
Value‑Based Care model focused on Geriatric patients
Small patient panel: Daily census of 10–15 patients
Emphasis on quality care over volume
No lengthy credentialing process
Key Responsibilities
Provide primary and preventive care services to elderly patients in an outpatient setting
Conduct physical exams, diagnose and treat acute and chronic conditions
Order and interpret diagnostic tests, prescribe medications, and manage treatment plans
Educate patients on health maintenance and disease prevention
Collaborate with physicians and other healthcare providers to coordinate care for PACE Participants
Maintain accurate, up‑to‑date patient records in the electronic medical record system
Compensation & Benefits
Compensation: $155,000 - $170,000
Annual bonus (up to 20% of salary)

Requirements
Current Nurse Practitioner or Physician Assistant licensure in New York State
National board certification (FNP, ANP, AGNP or NCCPA)
At least 1–2 years of clinical experience preferred (new graduates with strong clinical rotations may be considered)
Must have Medicaid and Medicare Provider ID
Excellent communication, organizational, and clinical skills
Ability to work independently while collaborating effectively with a care team
