Overview
RN Case Manager – Monday–Friday 8am–5pm. No call or weekends. No ad hoc calls or visits outside of case managing your patients. Sign-on bonus available; benefits Day 1. RN, Case Manager Key responsibilities
Assess and identify patient/family needs using the nursing process; coordinate the Plan of Care with the Interdisciplinary Team (IDT); provide clinical, palliative and supportive care to enable the patient to remain in their home environment as long as possible. Coordinate the Plan of Care ensuring it reflects the patient’s evolving needs; educate patients, families, and caregivers about disease processes, palliative interventions, caregiving, safety, and the dying process. Visit patients in their homes to assess home safety, medication and nutritional compliance, and durable medical equipment (DME) needs; ensure visits align with risk scores/needs. Report changes in the patient’s condition to IDT or other health professionals; participate in IDT collaboration meetings and provide concise oral and written reports. Initiate and supervise skilled nursing interventions to enhance prevention, alleviate symptoms, and maximize comfort; obtain physician orders and ensure documentation per Company policy. Act as a Company representative at assigned facilities, facilitate referrals to service lines, and work closely with hospitals, physicians, facilities, patients, and families; maintain frequent communication with IDT. Provide all necessary clinical communications timely using SBAR and discuss potential needs with after-hours staff. Develop strong relationships with case managers, physicians, and facility staff; supervise patient care provided by Community Health Workers and Home Health Aides as requested. During emergencies (e.g., hurricanes), may be required to report to a designated location to ensure continuity of services; may stay overnight based on emergency duration. Performs other duties as assigned. Qualifications
Current RN license in the state of employment. Minimum of one year nursing experience; hospice or hospital experience preferred. Experience with EMR/EHR systems. Mobile driver with valid license and automobile insurance per company policy; reliable transportation to meet visit schedule. Ability to use equipment with visual and auditory capabilities; effective English communication (verbal and written). Ability to visit participants in their homes and perform essential job functions including lifting patients/equipment; reasonable accommodations available. Active BLS for healthcare professionals from the American Heart Association or Red Cross. Other locations
Some locations may require additional responsibilities such as providing reassurance on the phone, coordinating in-person visits, and utilizing resources to resolve complex situations; documents patient/family contact information in the EMR and communicates with the IDT; completes initial and semi-annual assessments across Company services; explains services to patients/families and addresses questions, while maintaining empathy and sensitivity. Initiates Plan of Care and documents accordingly; participates in facility relations and QA activities. Competencies

Provide and manage direct care as part of the IDT, incorporating psychosocial, spiritual, cultural, physical, and biological components; implement appropriate nursing interventions and follow-up. Develop individualized Plan of Care reflecting evolving patient needs; educate patient, family, and caregivers about disease, prevention, palliation, caregiving, safety, and end-of-life considerations. Conduct home visits to assess safety, medication compliance, nutrition, and DME; determine visit frequency by risk score/needs. Document changes in patient condition; communicate with IDT and other health professionals; participate in daily IDT meetings. Provide concise oral and written reports to IDT; encourage multidisciplinary input; communicate accurately with physicians, staff, patients, families, and supervisors. Supervise patient care by Community Health Workers and Home Health Aides as requested. Offer flexibility during emergencies to ensure continuity of services and coordinate with the company’s designated response plan. Perform other duties as assigned. Physical demands
Physical requirements include seeing, hearing, talking, walking, and manual dexterity; frequent activities include standing, sitting, reaching, lifting, bending, and kneeling; occasional tasks include pulling, pushing, stooping, and stairs navigation.
