Job Summary:
The Supportive Services Coordinator will hold a caseload assigned through internal referrals from our testing and research projects. This candidate will manage retention in care for their caseload, follow fidelity monitoring protocols, and contribute to data collection and documentation. Expected to pursue professional development, stay informed about resources on the South Side of Chicago, and inform the team about collaborative opportunities. Position is grant funded.
Responsibilities
Calling community members and conducting resource counseling, completing documentation and notes, researching appropriate and individualized resources for people of color and those who identify as LGBQ and TGNC.
Work with community members over the phone or in person.
Assist with day‑to‑day Supportive Services tasks, rapid response support for same‑day treatment seekers, conduct assessments and safety plans for community members in distress.
Support evenings and weekend events where supportive services are requested.
Assist with and perform various administrative and operational tasks under direct supervision.
Use knowledge of clinical studies to coordinate collection of analyzable clinical research data and/or samples.
Perform other related work as needed.
Minimum Qualifications
Vocational training, apprenticeships, or equivalent experience in a related field (four‑year degree not required).
2–5 years of work experience in a related discipline.
Strong organizational, interpersonal, and communication skills.
Knowledge of Chicago social service resources and program subject matter.
Fluency with Microsoft Office (Outlook, Excel, Word).
Ability to manage multiple projects, set priorities, and meet deadlines.
Willingness to travel to community venues across the metro area.
Preferred Qualifications
Advanced degree in a related field.
Experience providing care to LGBT populations, Black and Latino community members, and HIV/STI treatment and prevention.
Demonstrated leadership and teaching skills.
Strong knowledge of program subject matter and Chicago resources.
Ability to perform phlebotomy or willing to be trained.
Creativity, flexibility, and ability to work non‑traditional hours.
Proficiency in data collection and monitoring protocols.
