GENERAL DESCRIPTION Responsible for monitoring effective strategies and initiatives to ensure proper coordination of daily operations impacting the Long-Term Care Home Program. Develops, promotes, and maintains a healthy business relationship with participating Long-Term Care Home Administrators in our network, ensuring satisfaction, retention, and new referrals. Serves as the primary liaison between the Company and the Home Care Administrator to address, channel, and resolve service issues related to barriers and operational situations that impact the provision of care to older adults.

ESSENTIAL FUNCTIONS

Assists in monitoring strategies and initiatives to achieve efficient home care service operations.

Assists in updating regular reports such as: Census, Sales, CHRAs, STARS, MTM, and urinary incontinence program.

Supports the development of Specialists by offering guidance, recommendations, and expertise in home service.

Plans, organizes, and conducts visits to home facilities to guide the administrator or staff member in charge according to current operational needs.

Serves as the liaison between home service providers and the Sales Department. Develops strategies and initiatives related to sales to increase and retain membership.

Analyzes the needs and root causes of situations in each care home administration to identify the root cause of service barriers. Refers issues to staff from other units and ensures they are resolved following established company policies.

Refers requests, inquiries, and identified potential risks to their supervisor and/or other departments for immediate handling and resolution.

Prepares, documents, and keeps records of situations reported by care home administrators and ensures they are resolved.

Participates in the research and implementation of strategies for offering services to care homes.

Establishes a work plan with leaders to establish service quality monitoring programs, strategies, initiatives, and educational activities aimed at strengthening relationships with administrators and keeping them informed of any changes.

Participates in educational and recreational activities as operationally necessary.

Must comply fully and consistently with all company policies and procedures, with local and federal laws as well as with the regulations applicable to our Industry, to maintain appropriate business and employment practices.

May carry out other duties and responsibilities as assigned, according to the requirements of education and experience contained in this document.

MINIMUM QUALIFICATIONS Education and Experience:

Bachelor's degree in Business Administration, Health Services Administration, or related area. At least five (5) years of experience performing similar tasks such as administration of health services programs, health care services, and/or providers administration, preferably within the Health Insurance Industry. "Proven experience may be replaced by previously established requirements."

Certifications / Licenses:

A Driver's License valid in the Commonwealth of Puerto Rico is required.

Other:

Knowledge of regulations applicable to the Health Industry and/or Medical Plans. Availability to work extended hours, weekends, and holidays, according to business needs and the requirements of regulatory agencies applicable to the Industry. Must be available to travel around the island, conducting home care facility visits.

Languages:

Spanish – Advanced (comprehensive, writing and verbal); English – Advanced (comprehensive, writing and verbal).
