Full-time Nurse Practitioner to join Cardiology team at 1580 Valencia Street, San Francisco, CA.
About the Opportunity The NP will manage follow-up visits for established cardiology patients, support transitional care with post-discharge visits, and provide patient education on cardiovascular conditions.
Key Responsibilities
Conduct follow-up visits for established cardiology patients.
Support transitional care management through post-discharge visits.
Provide patient education and promote medication adherence.
Collaborate with cardiologists and care teams to ensure continuity of care.
Document patient encounters in Epic.
Education Graduate of an accredited Nurse Practitioner (NP) program.
Qualifications
Current California NP license.
DEA Controlled Substance Registration Certificate (CSRC).
Current BLS and ACLS certification.
Experience in cardiology or internal medicine preferred.
New graduates with a strong interest in cardiovascular care are welcome to apply.
