Salary:
$71,500 – $78,000
Position Summary Commonwealth Care Alliance’s Long‑Term Services and Supports (LTSS) team manages relationships with community‑based organizations (CBOs) that coordinate services for CCA members. The LTSS Program Specialist provides project management, data analysis, training, and external partner support to ensure efficient CBO operations and high‑quality member care.
Responsibilities
Support LTSS network program operations and documentation.
Develop agendas, take notes, and follow up on action items for group meetings.
Schedule regular check‑in meetings with CBO staff and agencies.
Perform data quality checks and maintain reporting inventory.
Coordinate provider roster management with the Provider Data Networks Team.
Disseminate information to CBOs and provide guidance on SOPs and workflows.
Draft email communications under supervision of the Manager.
Manage onboarding process, including training, documentation, and access termination.
Track support tickets and manage assignment transfer logs.
Assist the LTSS Program Manager during audits and ad‑hoc trainings.
Qualifications
1+ years of experience in LTSS, provider relations, or network management.
Experience with dual‑eligible (DSNP) populations preferred.
Knowledge of Medicare and Medicaid (MassHealth) and community‑based services for seniors and disabled individuals.
Proficiency in Microsoft Office (Excel, PowerPoint) and ability to learn new systems.
Excellent written and verbal communication skills.
Strong organizational, analytical, and problem‑solving abilities.
Ability to build and maintain positive relationships with internal and external stakeholders.
English proficiency required.
Education
Bachelor’s degree (Public Health, Health Management, Social Work, or related field preferred).
Working Conditions
Remote or hybrid role with standard operating hours 8:30 am‑5 pm Monday‑Friday.
