Monday-Friday, 8:00am-5:00pm. No call or weekends required. Position Summary

The APP will coordinate with multiple surgical divisions to identify patients with diabetes who need surgery. The APP will perform an assessment and provide an individualized pre-habilitation framework to maximize health before surgery and teach lifelong skills for post-surgical health management. The provider will coordinate care with surgical teams, endocrinology support, anesthesia pre-operative teams, and a data abstractor to track patient outcomes. Responsibilities

Takes comprehensive history and performs physical examinations Evaluates and treats based on clinical data and guidelines Initiates referrals and consults with attending/collaborating physicians Documents and bills for direct care Utilizes current research and evidence-based decision making Participates in quality improvement activities and clinical research Supports accreditation, compliance, and regulatory activities Maintains professional practice through involvement in professional activities and continuing education Potential to precept students and newer team members Maintains licenses, certifications, and CNEs/CMEs as required Coordinate with multiple surgical divisions to identify patients with diabetes who need surgery Assess pre-operative patients and provide an individualized pre-habilitation plan Coordinate care with surgical teams, endocrinology, and anesthesia pre-operative teams Collaborate with data abstractor to document important information for outcomes Engage with MUSC leadership for prehabilitation program initiatives Qualifications

Required Training and Experience Completion of an accredited Physician Assistant program and current licensure or eligibility as a PA, or completion of an accredited Nurse Practitioner program and current licensure or eligibility as an APRN Minimum of 1 year of experience as a PA, FNP, or ANP Preferred Training and Experience Clinical Nurse Specialist in Advanced Diabetes Management Experience managing patients with diabetes or advanced diabetes management training CDCES qualification Licensure, Certifications, Registrations

Current and complete certifications required prior to start date: Basic Life Support (BLS). If applicable, DHEC and DEA license applications must be started prior to first day. Physical Requirements

Continuous requirements to perform job functions while standing, walking, and sitting. Various physical and dexterity requirements are described, including lifting up to 50 lbs unassisted and potential patient handling duties. Additional new requirements include ability to deal with stress, computer literacy, and ability to work rotating shifts and overtime as required. Equal Opportunity
