Job Title: Nurse Practitioner SupervisorJob Description We are seeking a dedicated Nurse Practitioner Supervisor with a passion for serving diverse communities and providing comprehensive care to adult patients. This role involves leading and supervising care plans, conducting wellness assessments, and ensuring effective transition of care for patients. Responsibilities

Perform annual wellness assessments for adult patients. Educate patients to ensure they understand their medical complexities and comorbidities. Conduct routine quarterly assessments for those with chronic medical illnesses. Provide acute care services and facilitate same-day access to participants. Assess and manage participants with acute changes in condition through face-to-face or telemedicine visits. Conduct post-hospitalization visits within 7-14 days of discharge, including reconciliation of medications and coordination with specialists. Formulate a comprehensive diagnostic list of current and past medical conditions using clinical knowledge and judgment. Review participant’s current symptoms, identify active diagnoses, and manage chronic problems or conditions. Act as the direct supervisor of care plan formulation and ensure proper delivery by each discipline for community PCP participants. Essential Skills

Minimum of two years in clinical nursing practice in a primary care setting, home care, care management, or related field. Minimum of one year of experience working with the adult population, or appropriate training if less experience. Strong communication skills. Basic computer skills in Microsoft Office applications. Additional Skills & Qualifications

Nurse Practitioner certification. Management skills. Experience in geriatric care. Work Environment

The position operates Monday through Friday at a TeamCare site, serving adult patients in a primary care setting. Job Type & Location

This is a Permanent position based out of Brooklyn, New York. Pay and Benefits

The pay range for this position is $155000.00 - $165000.00/yr. Benefit Guide available upon request Workplace Type
