RN Case Manager – Home Health Full Time (Days) When you join the Visiting Nurse and Hospice for VT & NH, you’ll become part of a dedicated team delivering outstanding home health and hospice services that enrich the lives of the people we serve. RN Case Managers at VNH screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.
Responsibilities
Screen, evaluate, re-evaluate, and treat patients
Coordinate and communicate treatment activities with interdisciplinary team members
Develop, review, and recertify care plans in collaboration with physicians and family
Advise other disciplines of discharges in a timely manner
Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems or progress toward desired goals
Communicate significant changes to the patient's physician and VNA colleagues
Document evaluations, treatment goals, and objectives
Teach the patient and caregiver self-care techniques
Provide medication, diet, and other instructions as ordered by the physician and utilize opportunities for health counseling
Qualifications
Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license
Graduate of an accredited school of nursing
Minimum 2 years of experience in Home Health
Strong communication skills and ability to work as part of an interdisciplinary team

Certifications BLS/BCLS required. Additional nursing certifications such as CPI, ENPC, RNFA, TNCC, AWHONN, NIHSS, STABLE, ASLS, NBSTSA, APHON, CMA, CCMA, RMA, C-EFM are preferred.
