Home Office is Senior TLC Center - Charlotte, NC; supporting their client population
Position Summary: As a Certified Nursing Assistant (CNA) with Senior TLC (STLC), you’ll play a vital role in supporting the health and well-being of older adults—both in their homes and at our Adult Day Health Center. You’ll provide hands-on personal care, assist with daily activities, and help maintain a safe, comfortable environment. Whether preparing meals, helping with mobility, or supporting group activities, your care will make a meaningful difference in the lives of our participants. You’ll work closely with nurses, supervisors, and other team members to ensure each individual receives compassionate, personalized support.
May be required to assist with patient care through our affiliated Home Health Agency as needed.
Key Responsibilities: Provide personal care such as bathing, dressing, feeding, toileting, and mobility assistance in-home and at the center Prepare simple meals and assist with serving meals based on participants’ dietary needs Perform light housekeeping tasks including laundry, grocery shopping, and errands as part of the care plan Support group activities and outings, helping participants stay engaged and active Take vital signs and monitor oxygen levels as directed by the care team Assist with self-administration of medications under supervision Document care provided and observations accurately Report changes in participants’ health or behavior to the clinical team Attend staff meetings and complete required training and competencies Participate in on-call rotation for home care (weekdays and weekends)
Skills and Competencies: Compassionate and respectful approach to elder care Strong communication skills (verbal and written) Ability to follow care plans and work under supervision Comfortable with lifting, transferring, and assisting participants with mobility Reliable, organized, and able to work independently Familiarity with chronic health conditions and geriatric care
Minimum Qualifications: High School Diploma or GED 1–2 years of CNA experience, including at least 1 year with elderly or frail populations Active CNA I or II listing with the NC Nurse Aide Registry No history of abuse, neglect, or misappropriation of property Valid driver’s license, insured personal vehicle, and dependable transportation Medically cleared for communicable diseases and current on immunizations Must meet STLC and CMS competency standards before working independently
Preferred Qualifications: Experience in home care or adult day health settings Familiarity with oxygen equipment and basic medical procedures Ability to support participants with chronic conditions and cognitive impairments
Health Requirements: Must be medically cleared for communicable diseases and up-to-date on immunizations Must be able to provide care to adult and geriatric populations
