The ideal candidate will have a strong background in acute care discharge planning and case management within a Level 1 trauma or large academic medical center . This is a day shift position (Monday–Friday, 8:00 a.m.–4:30 p.m.) with no weekends or on-call required. Schedule & Details: Shift: Days, 5×8-hour shifts Hours/Week: 40 Duration: 13 weeks Start Date: 10/27/2025 On-Call: None Weekend Requirement: None Holiday Requirement: As needed, rotated Responsibilities: Perform acute care case management focused on discharge planning Manage a caseload of 15–20 patients per day across inpatient units Collaborate with social workers , physicians , nursing , pharmacy , and RT for discharge readiness Utilize naviHealth for referrals and discharge coordination (transitioning to Epic Care Management ) Assist in safe transitions of care, including medical and social discharges Work in a Level 1 trauma center serving a diverse patient population Report directly to the Social Work Manager and collaborate with the Chief of Care Coordination Requirements: Active RN license (Compact or MS) 2 years of acute care case management experience in discharge planning At least 6 months in a large academic or Level 1 trauma hospital BLS certification (Required) EPIC charting experience (Required) CCM certification preferred (not required) BSN preferred (ADN considered) Flu vaccine required (medical exemption only with documentation) Charting System: Epic Systems Corporation – EpicCare Inpatient (EMR 2014 Certified EHR Suite) naviHealth (transitioning to Epic Care Management) Department Overview: Department: Case Management – Discharge Planning Team Structure: 15 Case Managers (goal: 21 total) 15 full-time 17 part-time Social Workers 3 Complex Case Managers Setting: Acute, inpatient units across the hospital Environment: Collaborative, interdisciplinary care model Contact HumanEdge Health: HumanEdge Travel Healthcare Job ID 12938.
