Position: Licensed Clinician at Insight Housing. The Hope Center provides 53 units of permanent supportive housing and related services for homeless adults and disabled individuals, including mental health support and case management. Responsibilities
Develop and maintain a respectful, caring, tolerant, ethical, and empowering relationship with all individuals served. Provide counseling and consultation to clients in collaboration with Hope Center Management & Case Management Teams. In partnership with the Regional Director of Residential, act as an agency liaison with AC Healthcare Services Agency (HCSA) in the use of the CHR. Assist in managing Community Healthcare Record (CHR) access and use across all Insight Housing programming. Collaborate with program managers and staff to coordinate on-site services and development of appropriate Housing Plans. Support a caseload of 10-15 clients, providing clinical interventions including mental health screening, assessments, and coordination with current providers; assist with program rules explanations and resident onboarding. Establish and maintain clear and effective communications with clients; develop and implement treatment plans and prepare warm hand-offs to housing providers or discharge plans. Provide informal counseling focusing on immediate needs and situations. Provide Cognitive/Behavioral interventions to help clients achieve goals, in collaboration with onsite team. Provide mental health guidance to onsite teams in weekly case conference meetings. Work with medical staff on hospital discharge planning and advocate for residents’ care needs. Provide crisis assessment and intervention; de-escalate as needed and interface with crisis/emergency responders when necessary. Provide training to Hope Center staff on mental health topics and trauma-informed care. Supervise one-two Clinical Interns annually, offering clinical supervision and support. Understand the 5150 process and collaborate with on-site staff and external providers as needed. Contribute to a safe, professional working environment free of hostility, harassment, and discrimination. Attend Alameda County meetings to collaborate on housing initiatives and resources. Drive the agency vehicle to various sites as required and document mileage per policy. Promote a safe, healthy, and clean working environment in line with health and safety practices. Attend meetings and trainings as assigned; submit timesheets timely and accurately. Adhere to Insight Housing’s Code of Conduct and perform other tasks as assigned. Qualifications, Skills, And Abilities
LCSW or LMFT required Ability to complete CPR/First Aid Training At least 2 years’ direct experience serving veterans or individuals experiencing homelessness, mental health, substance abuse, or related issues Ability to provide supervision Experience in psychiatric rehabilitation, preferably in a social rehabilitation environment Commitment to serving individuals experiencing homelessness with a trauma-informed approach Proficiency in computer use and Microsoft Office; HMIS experience is a plus Ability to interact professionally with diverse staff and clients Strong customer service, communication, and crisis intervention skills Ability to work independently and as part of a team; maintain confidentiality and professional boundaries Excellent verbal and written communication; ability to work under pressure Ability to manage multiple tasks and meet deadlines while maintaining professional conduct Special Requirements
Frequent driving required; valid California driver license and suitable MVR Criminal and credit records clearance required; TB clearance Physical Requirements
