Voluntary Case Management (VCM) program serves families on Oahu who have been referred by Child Welfare Services (CWS) after a report to the Child Abuse and Neglect Hotline and identified as moderate to high risk. Each family is assigned a case manager who addresses a range of family system issues that place children at risk of abuse and neglect. They may also be assigned a skill builder who will work with the family to increase skills such as parenting, co-parenting, problem solving, and stress management. The Case Manager provides voluntary, culturally appropriate, and strengths-based services related to child safety concerns to families referred by the Department of Human Services who are at moderate to high risk for child abuse and neglect. Services focus on addressing reported concerns, increasing parents’ ability to establish and maintain safety for their children, meet their children’s needs, problem solve, and access community resources. Services are provided at the home or other environments preferred by families. The Case Manager engages families, refers, coordinates services, plans, ensures children’s safety, provides parenting and family strengthening activities, conducts outreach, monitors progress and documents family status. This position is under the direct supervision of the Program Supervisor. Responsibilities
Ensure delivery of services aligns with funding agencies’ standards, state law, and Parents And Children Together policies and procedures. Provide assessment and lead case management to ensure families gain access to resources efficiently and in a timely manner. Assess each family’s strengths and weaknesses, identify needs, and develop goals and objectives as part of the Family Partnership Plan and related activities. Complete required assessments (e.g., Child Safety Assessment, Comprehensive Strength and Risk Assessment Tool, Adult-Adolescent Parenting Index) within required timelines. Provide social services including parenting skill building, child development education, alternatives to corporal punishment, enhancing problem-solving and support networks, and preparing for longer-term services to reduce risk factors and increase protective factors. Link families to community resources and conduct follow-up, ensuring all Family Partnership Plan activities are in progress or completed. Meet monthly with Program Supervisor to discuss progress; notify supervisor of progress, challenges, and critical issues; consult weekly on top-tier families or crises; engage in case consultations and obtain supervisor review/approval of reports. Complete clear, concise, non-judgmental written and electronic documentation (initial assessments, monthly updates, closing reports) in a timely manner for review and approval by the Program Supervisor. Minimum Qualifications
Bachelor’s Degree in Social Work, Psychology, or related human services field required. Other Requirements
Possesses and maintains a valid driver’s license and daily access to an automobile. Possesses and maintains proof of no-fault insurance. Can obtain an acceptable criminal background check upon hire, one year later, and annually thereafter. Can obtain an acceptable CWS background check upon hire, one year later, and annually thereafter. Can obtain an acceptable National Sex Offender Registry Check upon hire, one year later, and annually thereafter. Preferred Qualifications
One (1) year relevant working experience in child abuse and neglect case management services including comprehensive home visiting assessments, domestic violence, and substance abuse highly desired.
