ThePosition: We're looking for a passionate Resident Care Manager who oversees the daily delivery of care of our ARCH residents. Keeps Director of Clinical informed of both progress and potential problems. Coordinates community admissions, discharges, and transfers.
Plans, develops, and organizes work tasks; Coordinates ARCH services and activities in consultation with the Case Managers.
Plans andsupervises resident care. Continually evaluates residents'physical conditions and participates in resident care conferences.
Monitorsqualityindicatorsandensurespositive outcomestothe extentpossible. Actively participates in and supports continuous quality improvement efforts.
Participates in admission and discharge planning as the ARCH Case Manager.
Requirements:
Graduate of an accredited school of nursing.
Current Licensed Practical Nurse or Registered Nurse in the State of Hawaii required.
At least three (3) years' experience in a hospital or a long term care facility. Supervisory experience preferred.
Current and valid 2 Step TB clearance and health review.
FirstAidandCPR preferred.
