As a RN Clinical Liaison, you will educate healthcare professionals about hospice services and provide end of life solutions that are committed to clinical excellence, compassion and dignity when life matters most. Ensure intake and referral processes are performed effectively and timely to meet the needs of patients, families and referral sources. Complete admissions process on referred patients in emergency cases, as staffing indicates, and for inpatient hospice admissions. Provide accurate and timely information regarding hospice services to inquiries. Receive calls from referral sources, respond timely, and initiate paperwork to begin Intake/referral process. Serve as patient advocate in evaluating/developing plan of care and obtaining informed consent for hospice. Call on multiple account types – SNFs, ALFs, Hospitals and Physicians. Offer in-services and education on the benefits of hospice. Hospice RN Clinical Liaison Requirements
Hospice heart: compassionate, kind and empathetic to your clients, their patients and their families. RN license in the state(s) you will be employed. Minimum of 2 years of healthcare experience in hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planning, hospice case management or RN education.
