Project Kinship
Overview
Clinical Case Manager position located in Orange County, CA. Primary responsibilities include providing mental health services, coordinating with MDTs, case managers, and community partners to support re-entry of adults recently released from incarceration. This role requires a flexible schedule including weekends and evenings as needed.
Responsibilities
Maintain and oversee MDT ongoing caseload of a minimum of 20 clients for intensive case management and mental health services including risk assessment, short-term counseling, and follow-up treatment for adults who have been recently released from incarceration and re-entering the community.
Provide mental health services including risk assessment, short-term counseling and follow-up treatment for formerly incarcerated adults.
Work closely with case managers, substance use counselors and peer navigators to ensure adherence to treatment plans.
Collaborate with community organizations, county services and other wrap-around services to establish continuity of care for warm handoff linkages for clients.
Work closely with Program Coordinator and serve as leader within the clinical/MDT team, acting as lead in the absence of the Program Coordinator to support supervision of team members.
Provide back-up coverage for the Program Coordinator as needed.
Participate in staff meetings, case conferences, team meetings, triage and supervision sessions; share best practices and ensure standard compliance.
Conduct and attend trainings relevant to team and participant needs.
Submit requests to the Program Coordinator for support services appropriate to treatment plan goals.
Document and track client usage of such items in case files; maintain case files in a ready state for audits according to quality assurance and compliance standards.
Ensure mental health treatment services are provided in concert with program policies, procedures, government rules, and regulations.
Participate in trainings, perform assigned duties, and submit timecards/leave requests according to deadlines.
Ensure HIPAA, ethical code of conduct, and professional boundaries are maintained with staff and clients.
Implement management decisions in a positive and timely manner.
Commit to the agency’s Mission, Vision, and Values; represent actions that define the overall philosophy and objectives of the agency.
Provide clinical supervision to approximately two or fewer bachelor’s level interns; assign and monitor the balance of clinical and case management caseloads to support intern productivity.
Have a broad understanding of adult re-entry services, prioritizing mental health services, substance use disorder assessment, housing-related assistance, case management, peer support, recovery services, life skills, job skills training and civil legal services.
Work a flexible schedule outside of business hours, including weekends and evening hours, as needed to support the program and leadership.
Compile and submit weekly units of service, demographics, and other required evaluation data to the Data Analyst and Director of Programs in a timely manner.
Adhere to policies regarding professional ethics, appropriate boundaries, and client confidentiality.
Other duties as assigned.
Education And Experience
Master’s Degree in social work or related field, preferred; must be enrolled and currently in MSW/MFT or related Master’s level program in behavioral sciences/social work field.
At least 3 years of experience working within mental health programs, especially with gang-involved, incarcerated, recovery, severe mental illness, and/or re-entry populations and their families.
Minimum of two years of experience providing direct clinical counseling, case management, and knowledge of rehabilitation philosophy and trauma-informed best practices.
Qualified to provide supervision to Bachelor's level Social Work Interns is preferred.
Knowledge of DSM-V and working principles of recovery with adults.
Must complete a Live Scan Background Check.
Experience in county contract reporting requirements is preferred.
Intermediate PC skills; experience with Electronic Medical Record systems preferred.
Bilingual in Spanish or other threshold languages is preferred.
