Responsibilities The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide professional nursing services to patients/residents at home. The nursing process includes documentation of human responses to health problems in the home health environment. Activities include dependent, interdependent and independent functions; collaboration with patients/residents, families and other inter-professional colleagues (e.g., physicians) to plan, implement and evaluate care; practicing within the full scope of nursing expertise; care across all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up in the home setting; and coordinating care, patient/resident assessment, education, triage and other nursing interventions.
Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure. Adheres to the Code of Ethics for Nurses as published by the American Nurses Association.
Qualifications
Graduate from a nationally accredited nursing program (CCNE, ACEN, NLN CNEA) preferred; Bachelor's degree in nursing preferred.
Minimum of one year of professional nursing experience preferred.
Currently holds an unencumbered RN license with the State Board of Nursing or multistate licensure if in an NLC state.
Obtains and maintains required department competencies and certifications.
Valid driver's license may be required depending on location.
Basic Life Support (BLS) required.
COS-C (OASIS Specialist - Clinical) preferred.
