Clinic Triage Registered Nurse Location:
Upland Hills Health - UHH Family Practice Clinics (primarily in the Dodgeville Family Practice clinic; may work at other Family Practice clinics as needed) Role & Department:

Full-time (1.0 FTE) day shift, Monday through Friday 8:00 a.m. to 5:00 p.m.; no weekends or holidays Posting date:

Responsibilities
Under general supervision of the Clinic Management Team and the Clinic Physicians, perform care coordination and professional nursing functions to assess the severity of patients’ health concerns using protocols, and provide dispositions to emergency care, clinic provider appointments, specialty care, and triage nurse clinic services within the scope of the licensed practice. Support providers in triaging walk-in patients as well. Be part of a care coordination team supporting general care processes alongside other clinic personnel and providers. Manage nurse-directed patient telephone calls and triage requests from walk-in patients seeking health care services. Evaluate and assess patients’ health care needs based on signs and symptoms using established triage protocols and guidelines. Obtain input and direction from the provider as needed to determine the best clinical course of action and recommendations for specific patient health care needs. Accurately document symptoms/complaints, nursing assessments, advice provided, and patient/caller response. Follow policies, procedures, and protocols to ensure consistency and improve health care outcomes and patient access to care. Coordinate and schedule high-priority same-day and walk-in appointments on the provider schedules. Utilize standing orders, triage protocols, and algorithms appropriately. Monitor patient flow in the clinic to ensure patients receive all recommended services in a timely manner. Perform other nursing functions as assigned and assist providers with special procedures as needed. Manage prescription refills per protocol. Provide patient education on chronic disease management, prevention, and family health; train new staff in this role and its duties and responsibilities. Participate in Performance Improvement activities as assigned. Ensure pertinent medical information is forwarded to proper departments for continuity of care. Document in the electronic health record (EHR) including triage assessment, plan, implementation, evaluation, and follow-up. Participate in organizational and departmental CQI activities.
Qualifications
Candidates must have a current Wisconsin (WI) unrestricted RN license and current CPR certification. Prior clinic-related triage experience of at least 1 year is preferred. Prior experience working in a medical clinic is highly preferred. Knowledge of disease processes and normal growth and development across all age groups. Community health knowledge and theory; ability to work with diverse populations. Advanced computer and software skills; experience with the EPIC EHR system is preferred. Nursing assessment skills with/without visual inspection using basic nursing theory; critical clinical thinking skills. Excellent communication skills (listening, and written/spoken English) when interacting with patients, families, health professionals, and community agencies. Capacity to function independently within licensed scope of practice. Ability to handle stressful emergency situations calmly and effectively. Proficiency with Microsoft Office Suite is beneficial.
