Perform all duties of a staff pharmacist by conducting direct patient care services. Coordinate and help manage clinical pharmacy patients, programs, and initiatives, including but not limited to; performing professional clinical reviews of clinical protocols needed for patient care, conducting Medication Therapy Management (MTM), administration of immunizations, Remote Patient Monitoring, Chronic Care Management, Adherence Programs, Disease State Management services, Point of Care Testing and other clinical services as assigned. Team Leadership: provide leadership to pharmacy staff and collaborate with management to meet goals. Provide a pharmacy atmosphere and excellent customer service to develop and maintain patient and employee loyalty. Leads team to meet and exceed financial goals including payroll, sales, and clinical service expectations. Leads team by modeling, coaching, and maintaining high standards in patient service. Leverages Regional Support Pharmacy Manager, Regional Support Pharmacist, and Senior Regional Support Pharmacist for development. Organizes and directs workflow. Provides professional services to patients by presenting a positive image of leading by example. Manages pharmacy and clinical services according to payer, state, federal and company regulations to ensure compliance. Maintains a clean, neat, and well-organized work area. Develops and follows up on long term care (LTC) business through communication with nursing homes, group homes and assisted living facilities. Fosters development of the clinical services. Supports cross-functional teams by modeling, coaching, and maintaining high standards in patient service. Clinical Services: supports and completes all Pharmaceutical Care responsibilities including drug administration, point-of-care testing, patient consultation, disease state management, medication therapy management, immunizations as allowed, and other clinical services as assigned. Delivers MTM services including Comprehensive Medication Management, Comprehensive Medication Reviews (CMR), Targeted Medication Reviews (TMR), Disease State Management, and assessing medication appropriateness to optimize therapy and outcomes. Uses evidence-based practices and current clinical guidelines to formulate recommendations and treatment plans. Maintains clinical rules and protocols in internal software systems; individualizes patient care by evaluating and documenting clinical responses to therapy. Performs patient counseling in compliance with protocols and documents interactions with patients and prescribers in appropriate systems. Conducts patient-centered adherence services and collaborates with prescribers and care teams to set disease-state goals. Coordinates care to meet patient outcomes including referrals, medication review and delivery systems, and discharge planning from services. Performs CLIA-Waived Point-of-Care Testing per protocol and integrates POCT into patient care with QC procedures. Provides oversight to pharmacy technicians or interns delegated POCT responsibilities where allowed by state law. Prescribes medication according to Collaborative Practice Agreements. Serves as an up-to-date drug information resource for staff. Engages patients through quality interactions and relationship-building. Collaborates with Long Term Care pharmacy teams on clinical service initiatives. Promotes immunizations in-store and off-site as needed and with local employers and long-term care facilities. Participates in local health fairs and serves as an information resource for the community as directed by the company. Contributes to quality improvement initiatives and performance metrics related to patient care and clinical services. Addresses open quality gaps with patients and collaborates to close them. Additional Requirements

Working knowledge of third-party pay plans and quality measurement entities such as CMS and PQA Excellent customer service and communication skills Proficient in the English language Maintain patient confidentiality at all times Attention to detail and ability to follow policies and procedures Proficient with Microsoft Office and basic computer skills Ability to handle stressful situations and work with difficult customers Ability to maintain effective working relationships with employees and supervisors Understanding of Continuing Education requirements and responsibility to meet them Reliable attendance and punctuality Availability to work evenings, weekends, and holidays when needed Meets productivity and performance expectations Required Qualifications

Education

B.S. Degree or Pharm.D. from an accredited institution Experience

Three years of experience as a practicing pharmacist Capability to evaluate and monitor drug therapy in ambulatory care in collaboration with physicians, nurses, and other healthcare professionals Licensure/Certification

Current Minnesota pharmacist license Current or willing to obtain North Dakota, Iowa, and/or Wisconsin pharmacist license Certificate of training for Immunizations CPR or BCLS certification Preferred Qualifications

Education

PharmD Two years of professional experience as an Ambulatory Care Pharmacist or equivalent Clinical background in Ambulatory Care Licensure/Certification

Board of Pharmacy Specialty (BPS) Certification Licensed as a pharmacist in multiple states where the organization operates; current licensure is a requirement Physical Demands

In-office work with potential long periods of standing and sitting; ability to stand, talk, hear, and operate tools and controls; frequent use of hands Occasional lifting up to 40 pounds; vision requirements include near/far/color/depth perception
