Overview
Team Lead, Transition Liaison - Full Time First Shift at Valley Health. This role supports discharge planning to Home Health and coordinates clinical assessments for patients requiring Home Health.
Responsibilities
Assist the RN Transition Liaison in daily role and processes to organize and facilitate thorough discharge plans to Home Health.
Complete clinical assessments for patients requiring Home Health.
Assist Home Health DME technician by ordering and delivering DME to patients’ rooms to facilitate a safe and timely discharge.
Interview patients, family members, and/or caretakers as needed to complete assessments.
Maintain open communication with the care team regarding approval/denial of Home Health assessments.
Maintain proficiency in Home Health requirements based on regulatory and procedural processes.
Assist with arranging and following up with community resources as needed.
Support a seamless transition utilizing the continuum of care and movement to Home Health.
Extra Duties
Assisting Home Health staff with completion of re-hospitalization assessment.
Education Graduate of an accredited school of nursing required.
Experience
Two years bedside experience required.
Two years Case Management preferred.
Certification & Licensure
Licensed Practical Nurse in the states of Virginia and West Virginia required.
BLS Certification (Basic Life Support) - American Heart Association Healthcare Provider (HCP) required. New hires must have AHA-certified prior to orientation completion.
