Provides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policy. Serves as a resource to other healthcare professionals including pharmacy students and residents and ensures patient care meets standards of safety and effectiveness. Oversees the duties of the pharmacy technicians and interns. Responsibilities and Requirements
Primary Responsibilities: Ensure safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable. Documents medication-related plans and interventions in electronic medical record. Provide clinical consultation delivering best-practice, evidence-based information to practitioners. Make recommendations and encourage safe and cost-effective pharmacotherapy through multidisciplinary collaboration. Oversee the medication dispensing process to ensure accurate and timely delivery of pharmacotherapy, including supervision of pharmacy technicians and/or interns. Precept pharmacy students and residents, as applicable. Work in a constant state of alertness and safe manner. Perform other duties as assigned. Education
Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacy Experience

Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements. Visual and depth perception requirements for distances near and far and color differentiation. Hearing and speech abilities to communicate and respond to alarms or malfunctioning machinery. Frequent keyboard use/data entry. Occasional bending, stooping, kneeling, squatting, twisting and gripping. Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs. Rare climbing. Licenses and Certifications
Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 days of employment. Other resuscitation certifications may be required depending on department. Pharmacist Licenses by state (Illinois IDFPR, Missouri DPR, Oklahoma Board of Pharmacy, Wisconsin DSPS) as applicable to work location. Work Location
