Summary Under the general supervision of the Program Manager, provide high-quality Intensive Case Management Services (ICMS) to clients eligible for permanent housing through the Department of Health Services’ (DHS) Housing for Health (HFH) and other programs. Use a client-centered approach and deliver customer service that is sensitive to the challenges faced by people experiencing homelessness with medical and behavioral health issues as they move into and maintain permanent supportive housing. Employ a “whatever it takes” approach to assist clients in their transition from homelessness to permanent housing.

Essential Duties and Responsibilities

Assist individuals at every stage of the housing stabilization process, including flexible services to meet individual needs.

Outreach and engagement: process referrals from DHS, CES partners, and HFH partners; establish rapport with clients; conduct screenings; obtain consents; link clients to needed providers and services.

Conduct intake and enrollment activities, assist with program eligibility documentation, complete project intake forms, authorization, and enroll into ICMS.

Assist with temporary housing referrals until permanent housing is secured.

Ensure documentation is prepared and entered into appropriate systems on a monthly basis.

Assist at all levels of move-in to permanent housing; help clients become document ready; coordinate move-in and orient new tenants to their unit/building.

Assist clients with access to health, mental health, and substance use disorder services; conduct DHS-approved comprehensive psychosocial assessments; develop and implement individualized service plans with client input.

Ensure clients have access to medical homes and maintain continuity of care.

Assist clients with income and benefits, employment and education activities, basic needs, life skills, budgeting, and referrals to overcome barriers to securing permanent housing; provide transportation as needed.

Provide housing location services (if scattered-site) and educate clients on tenant rights and responsibilities.

Educate clients on appropriate use of crisis intervention services versus 911 calls; provide eviction prevention counseling and collaborate with property management and HFH partners to address housing stability issues.

Complete all paperwork in compliance with applicable laws and funding requirements; document in EHR with 24-hour turnaround when applicable.

Engage in ongoing professional development to maintain and enhance skills.

Position Requirements

High school diploma or GED with a minimum of 1 year of experience providing psychosocial rehabilitation related services to a disabled population.

Bachelor’s degree in a human service or related field preferred.

Valid driver’s license (Class C), access to a vehicle, insurance, and eligibility to be insured by MHALA liability insurance.

Proficiency in Microsoft Word, Excel, and Outlook; ability to learn HMIS and EHR systems.

Experience working with clients with mental illness, chronic health issues, and substance use disorders.

Knowledge in areas including homelessness, outreach and engagement, intensive case management, crisis intervention, housing and benefits applications, eviction prevention, and related services.

Ability to maintain a caseload of 20–25 clients as an intensive case manager (VI-SPDAT of 1 or 2).

Flexible work schedule, including some early mornings, evenings, weekends, and holidays.

TB clearance and yearly re-test are required; vaccination policy may change in the future.

Employment verification from three past employers within the most recent seven years.
