Role Description

This is a full-time on-site role for a Nurse Practitioner or Physician Assistant located in Wausau, WI. The NP/PA will deliver comprehensive primary care services to older adults in senior living communities and skilled nursing facilities. Day-to-day tasks include conducting physical exams, diagnosing and treating illnesses, prescribing medications, and developing care plans. The role also involves coordinating with other healthcare professionals to ensure holistic and patient-centered care.

Responsibilities

Deliver comprehensive primary care services to older adults in senior living communities and skilled nursing facilities. Conduct physical examinations, diagnose and treat illnesses, and develop patient care plans. Prescribe medications in accordance with state laws and clinical guidelines. Coordinate with interdisciplinary teams to ensure holistic, patient-centered care. Maintain accurate and up-to-date EHR documentation; ensure quality and compliance with regulatory standards.

Qualifications

Master's degree in Nursing or Physician Assistant studies from an accredited program. Valid Nurse Practitioner or Physician Assistant license in the state of Wisconsin. Certification from a recognized certifying body (e.g., ANCC, AANP, NCCPA). Basic Life Support (BLS) certification. Excellent clinical skills in conducting physical exams, diagnosing and treating illnesses. Proficiency in prescribing medications and developing comprehensive care plans. Strong communication and interpersonal skills for interacting with patients, families, and interdisciplinary teams. Experience in geriatric care or working with older adults. Ability to work independently and manage time effectively in a fast-paced environment. Proficiency with electronic health records (EHR) systems. Experience in value-based care models is a plus.
