NOW HIRING:

Full Time Wound Care Nurse

Summary/Objective In keeping with our organization’s goal of improving the lives of the Residents we serve, the Wound/Treatment Nurse plays a critical role in providing superior customer service and nursing care to all Residents. The Wound Nurse is responsible for performing skin treatments for all Residents under their care. This position will obtain treatment orders from attending physicians and assist with modifying the treatment regimen in accordance with established policies and procedures. The Treatment Nurse will provide supervision of staff and will safeguard the health, safety and welfare of all Guests under their care by following applicable laws, regulations, and established nursing policies and procedures.

Daily

Complete a head-to-toe body audit on all new admissions and re-admissions to ensure that all open areas have been identified by the admitting nurse. Ensure skin sheets, treatment orders, and preventative measures are in place (T&P, specialty mattress, heels up ordered or boots).

Weekly

Update skin sheets every 7 days. Choose a particular day and ensure updates by close of business on that day.

Unit Managers to update care plans weekly for pressure and non-pressure areas. Care plans updated every Friday.

Wound Report sent to VP of Clinical Services by Thursday with the percentage of Center Acquired Pressure Ulcers; goal is 3% or less.

Treatments are to be completed by Wound Nurse for vascular, surgical, and pressure injuries of stage 2 or greater (Monday-Friday).

Skin Tears, stage 1, to be treated by the Charge Nurse (Monday-Friday) with weekly skin sheets. Unit Manager and Wound Nurse are responsible for ensuring completion of these sheets weekly.

On-going

Wound Nurse is responsible for preventing wounds and providing daily education to staff on T&P for residents (in wheelchairs, geriatric chairs, and in bed), toileting, and incontinent care. Use a proactive approach to wound care and hold nurses and CNAs accountable for preventative maintenance.

When wounds worsen, treatment should be adjusted within a 2-week period with input from the wound physician and facility PCP.

Preventative items for residents at risk of Center Acquired Pressure Ulcers: orders for T&P, preventive barrier cream for all residents, barrier cream with zinc is beneficial, pressure-relieving devices for chairs, gel cushions for wheelchairs/geri chairs, Braden assessments on new admissions/re-admissions and any condition change. At-risk care plans should be in place for all residents. Rounds by UMs/charge nurses and wound nurses to ensure proper positioning, T&P every 2 hours, prn in bed and wheelchairs, heels elevated, and no skin-to-skin contact.

Consults with Nursing team for assessment evaluations and assisting in planning and developing skin care treatment.

Initiate requests for consultation or referral.

Assess residents and records on admission to distinguish normal from abnormal findings and determine when to refer to a physician for evaluation.

May act as a Unit Nurse when directed.

Provide supervision to CNAs and subordinate staff, ensuring assignments are completed.

Administer medications within the scope of RN or LPN licensure and maintain a current narcotics log.

Document contemporaneous charting and monthly evaluations of each resident’s condition and progress.

Ensure care plans are followed and assess each Guest’s status according to their care plan.

Round with wound physician weekly and PRN with physicians.

Be knowledgeable of individual care plans and support the care planning process by reporting resident needs, preferences, and behavioral changes.

Provide wound care when needed and delegate PRN tasks.

Complete weekly skin sheets.

Maintain infection control and standard precautions; follow safety practices when performing tasks and using equipment and supplies.

Maintain comfort, privacy and dignity of residents; interact with warmth and respect.

Respond promptly and courteously to call lights.

Assist with new admissions and re-admissions when directed.

Communicate effectively with residents, visitors, families, peers and supervisors.

Attend departmental meetings and in-services as directed.

Required Education and Experience

Graduate from an accredited nursing program.

Current State License as a Registered Nurse or Licensed Practical Nurse.

CPR Certified.

Wound care certification preferred or complete Vohra wound care certification within 30 days of hire.
