Physical Therapist (PT) - Mobile Outpatient role at Powerback. This position focuses on outpatient rehabilitation services provided in the home or in a resident’s room within independent living facilities, delivering patient-centered care to older adults and supporting overall rehabilitation and wellness goals. Powerback is a new model of delivering recovery, rehabilitation, respiratory, and wellness services nationwide. The Physical Therapist will primarily provide outpatient rehabilitation services in an in-home setting for older adults, including in private residences or in independent living facility rooms. Responsibilities include evaluating and treating in the most functional environment, developing and delivering PT programs to facilitate rehabilitation, referring to other disciplines as needed, and implementing individualized therapy plans to restore, reinforce, and enhance performance. The PT educates patients and caregivers to promote independence and productivity, directs activities to maintain health, and supports a flexible schedule in various settings. Specific duties include: Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel per state licensure and professional standards (as applicable). Adhere to department and patient treatment schedules; complete timely clinical and billing documentation; attend related meetings. Participate in patient care and staffing conferences; make referrals to other disciplines as needed. Collaborate with rehabilitation team to select appropriate therapeutic regimens; adapt plans of care to patient needs. Promote safety and eliminate hazards; order supplies and equipment as necessary; maintain and report on equipment needs. Provide patient-first service with compassionate care respecting patient rights and needs. Screen, examine, and evaluate patients; develop goals; plan and document treatment; deliver therapy including: Therapeutic exercise (aerobic conditioning, balance, gait, strength, flexibility, etc.). Functional training, manual therapy, airway clearance techniques, integumentary care. Use of electrotherapy, physical agents, and mechanical modalities; health and wellness education; chronic disease management. Falls risk assessment and home/community mobility interventions; case management; adaptive devices and orthotics/prosthetics; safety and mobility training. Evaluate progress and modify treatment plans; incorporate wellness programming; provide home and community mobility assessments with safety recommendations. Perform other related duties as required. Qualifications
Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy; or Master’s in a PT-related field with a Bachelor’s in Physical Therapy; or Bachelor’s in Physical Therapy with 5 years of progressive PT experience. If earned outside the U.S., degree must be evaluated as equivalent to a U.S. PT degree. Must have a valid driver’s license or reliable transportation to patients’ residences.
