Responsibilities

The Physical Therapist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient’s private residence or a patient’s room in an independent living facility. The Powerback Rehabilitation To You model allows the Physical Therapist to evaluate and treat in the most functional environment. The Physical Therapist assesses needs, develops, and delivers physical therapy programs to facilitate rehabilitation. They identify needs for and make referrals to other disciplines to address comprehensive patient needs. They implement individualized therapy programs to restore, reinforce, and enhance performance. They train patients and caregivers in skills for promoting independence, and direct activities to promote and maintain health. This position offers a flexible schedule and work environment across various settings. Supervision of Physical Therapy Assistants, temporary licensees, students and support personnel as appropriate to licensure requirements. Adherence to department and patient treatment schedules, timely clinical and billing documentation, and related meetings. Attend and contribute to patient care, staffing conferences and other meetings. Make referrals to other disciplines and agencies to address patient needs. Collaborate with rehabilitation team to select appropriate therapeutic regimens. Adapt program and/or plan of care to individual patient needs. Promote a safe environment and enforce safety practices. Order supplies and equipment as necessary; maintain and report equipment needs. Ensure high-quality patient service in a caring, compassionate atmosphere. Screen, examine, and evaluate patients; develop diagnoses, goals, and plans of care; document care. Therapeutic interventions include: therapeutic exercise, functional training, manual therapy, airway clearance techniques, integumentary repair, electrotherapy and mechanical modalities, health and wellness, chronic disease management education, falls risk reduction, home/community case management, adaptive devices and orthotics/prosthetics, mobility activities, and progression of treatment plans. Perform routine reexaminations and adjust plans as needed; incorporate health and wellness into treatment plans. Conduct home and community mobility assessments and provide safe mobility recommendations. Perform other related duties as required. Qualifications

Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy; or Master’s degree in a related field with a Bachelor's in Physical Therapy; or Bachelor's in Physical Therapy with 5 years of PT experience. If degree earned outside the U.S., the credential must be evaluated as the U.S. equivalent. Must have a valid driver’s license or reliable transportation to and from patients’ residences.
