Powerback is a new way of delivering recovery, rehabilitation, respiratory, and wellness services to patients across the nation. We deliver an empowered approach to achieving your full potential by providing integrated and individualized solutions. Our patient-centered care focuses on transforming the road to recovery by helping patients get their power back. The Physical Therapist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient’s private residence or in a patient’s room in an independent living facility. Powerback Rehabilitation To You allows the Physical Therapist to evaluate and treat in the most functional environment which is the patient’s home. The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. They identify the need for and make referrals to other disciplines to address comprehensive patient needs. They implement individualized therapy programs designed to restore, reinforce, and enhance performance. They instruct, educate and train patients and caregivers in essential skills to promote independence and productivity. Supervision of Physical Therapy Assistants, temporary licensees, students and support personnel as applicable per state licensure requirements and professional standards. Adherence to department and patient treatment schedule, timely clinical and billing documentation, and related meetings. Attend and contribute to patient care, staffing conferences and other related meetings. Make referrals to other disciplines and agencies to address comprehensive patient needs. Collaborate with rehabilitation team to select appropriate therapeutic regimens. Adapt program and/or plan of care according to individual patient needs. Promote a safe environment and enforce safety standards. Order supplies and equipment as necessary. Clean, maintain, identify, and report equipment needs or repairs. Put patient service first to ensure high-quality, compassionate care. Screen, examine, and evaluate patients, including history, systems review, tests and measures, and diagnosis; establish goals and plan of care; document all services. Therapeutic modalities include: Therapeutic exercise, functional training, manual therapy, airway clearance, integumentary care, electrotherapy and mechanical modalities, health and wellness, chronic disease management, falls risk reduction, and home/community case management. Designs/fits/adapts devices (including orthotics/prosthetics) and trains patients in their use. Selects activities suited to the individual’s capacity and interests; instructs and evaluates progress; modify plans as needed. Performs routine reexaminations to progress plans of care and integrates wellness programming into treatment. Performs home and community mobility assessments with safety recommendations. Performs other related duties as required. Qualifications
Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility Must have a Master’s degree in Physical Therapy, or a Master’s in a related PT field + Bachelor’s in Physical Therapy, or a Bachelor’s in Physical Therapy with 5 years of progressive PT experience Licensing evaluation for degrees earned outside the U.S. to determine U.S. equivalence Must have a valid driver’s license or reliable independent transportation to patient residences Seniority level: Entry level Employment type: Full-time Job function: Health Care Provider
