Responsible for the direct and indirect nursing care of the patients in the home. In collaboration with the patient, family, and health care team (IDT), the registered nurse as case manager will develop, implement, and evaluate nursing care according to agency and system policies. Performs the functions of the home health aide. Performs and monitors basic patient care according to policies and procedures. Provides a safe and comfortable environment in the patient’s home. Ensures privacy and demonstrates concern for the dignity of patient in performing all patient care. Completes assignments in a timely manner. Assumes on-call responsibility per policy and procedures. Communicates patient care problems or potential problems to the Administrative Staff and/or other interdisciplinary team members to assure continuity and coordination of care. Provides care based on physical, psychosocial, communication, safety, education level, and related criteria for infants, pediatric, adolescent, adult, and geriatric patients per established age-specific education/standard. States and utilizes admission criteria per agency guidelines. Completes OASIS documentation within federal guidelines. Completes and documents full system assessment, including assessment of physical and psychosocial aspects and identifies age-specific criteria upon admission and at each visit thereafter. Reports significant observed changes in patient condition to Administrative Staff and/or physician, documents patient status and care or services furnished, and documents home health aide supervisory visits as per standard. Coordinates care so that medications are administered as ordered by a physician and in compliance with medication administration guidelines. Coordinates the care team to facilitate changes in the level of care in accordance with change in level of care standards. Identifies the stages of death and dying. Accurately and timely completes plan of treatment and recertification as per Medicare guidelines. Completes OASIS SOC/ROC, OASIS Recertification, and Discharge OASIS per guidelines. Qualifications

Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the Enhanced Nurse Licensure Compact (eNLC). Valid driver’s license. CPR certification within 30 days of hire date. Experience

One year of Home Care Nursing and 2 years of prior nursing experience (preferred). Physical Requirements

Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling are necessary movements. Must be able to lift, move, and position patients of all weights with varying assistance. Must be able to read and write legibly in English. Hearing within normal range is required; visual acuity must be within normal range. Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly; physical demands are in excess of those for Light Work. Must be able to operate a motor vehicle with safe practice. Working Environment

Exposure to bodily fluids, human tissues, contagious diseases, sharps, and explosive gases. Exposure to toxic gases, fumes, and odors. Exposure to high stress and frequent interruptions. Exposure to electrical current and radiation from x-rays. Exposure to bad road conditions and adverse weather; extensive travel may be required as most work is performed in the patient’s home. Subject to unpredictable situations and potentially adverse home environments. Skills And Abilities

Ability to perform concentrated and complex mental activity with frequent involvement in highly technical situations. Ability to work under highly stressful conditions and adapt to varying workloads. Ability to make sound independent judgments and collaborate with a multidisciplinary team. Ability to comprehend and perform oral and written instructions and procedures. Strong written and verbal communication skills. Math skills to enable calculation of drug dosages and infusion statistics. Ability to work weekends, holidays, and all shifts. Demonstrates knowledge of and compliance with safety policies and procedures. IV skills required. Additional Job Description
