Clinical Pharmacist Full Time Nights – HCA Healthcare
Location: Commerce Township, Michigan | Facility: DMC Huron Valley-Sinai Hospital
Shift: Night (9:00 PM – 7:00 AM) | Employment type: Full Time
Overview
Under limited supervision, provides clinical pharmacy services including primary care services, consultative services, and drug information services to patients and health care providers to deliver high quality care.
Responsibilities
Review prescriptions as written by authorized prescribers to determine that orders are safe and appropriate for the specific patient; contact authorized prescribers as necessary to review vague or illegible prescriptions.
Fill prescriptions as ordered by authorized prescribers by dispensing or compounding medication according to established procedures.
Interview patients for prior medication history and provide counseling on current drug therapy. Identify patients who require teaching and provide educational material.
Collaborate with other health care providers to assure coordination of patient education; assign, educate, train and supervise the work of support personnel (technicians, residents, interns, clerks and students) in preparation of medications and related duties.
Ensure accuracy of work by support personnel in compliance with established policies and procedures; maintain inventory of medication and pharmacy supplies and related records.
Ensure storage of medications and supplies complies with environmental and regulatory agency procedures; provide formal patient and staff educational programs.
Conduct patient care area inspections to ensure proper drug availability, storage and dating; provide clinical pharmacy services in areas including therapeutic drug monitoring, anti-coagulation service, nutritional support, investigational drug services, hematology/oncology pharmacy, medication use evaluation, pain management service, and adverse drug reaction reporting.
Adhere to system, hospital and departmental policies, procedures, and objectives.
Qualifications
Bachelor of Science in Pharmacy or Doctorate of Pharmacy from an accredited College of Pharmacy or completion of foreign equivalency examination.
Registered Pharmacist and Controlled Substance licensures in the State of Michigan obtained within 3 months of entry into the job.
