Responsibilities

Conduct in-home assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam

Perform evidence-based practice screenings including point of care testing (as appropriate)

Identify diagnoses to be used in care management and active medical management for treatment

Formulate a list of current and past medical conditions using clinical knowledge, judgment, and assessment findings

Communicate assessment findings to inform the member's PCP of potential gaps in care

Identify urgent and emergent situations and intervene appropriately

Educate members on disease processes, medications, and adherence

Required Qualifications

Active NP license in the state of Oklahoma; licensure must be unencumbered with no open or unresolved disciplinary actions

Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care

Ability to meet physical requirements, including lifting a 30-pound bag, navigating stairs, and performing physical assessments while seated or standing

Reliable transportation to complete home visit assessments

Preferred Qualifications

Experience in gerontology, cardiology, internal medicine, or endocrinology

Home health care or home visit experience

Pay is based on factors including local labor markets, education, experience, and certifications. In addition to salary, benefits include a comprehensive package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (eligibility applies). The hourly pay range is $43.22 to $77.21 per hour based on full-time employment. Optum complies with all minimum wage laws as applicable.
