Provide direct patient care as a Nurse Practitioner or Physician Assistant in skilled nursing and senior living facilities. Perform comprehensive assessments, develop treatment plans, and provide ongoing care management focusing on preventive care and chronic disease management. Conduct regular rounds to assess residents’ conditions, adjust treatment plans, and address emergent health concerns. Coordinate care with facility staff, families, and other healthcare providers to ensure comprehensive, coordinated care tailored to residents’ needs. Educate residents, families, and facility staff on disease prevention, wellness, and management of chronic illnesses. Maintain accurate, timely documentation in the EMR and ensure compliance with regulatory standards and organizational protocols. Participate in quality improvement initiatives to enhance care delivery, reduce hospital readmissions, and optimize patient satisfaction. What We Offer

Valid Nurse Practitioner or Physician Assistant license in state of operation and current board certification in your field Master’s Degree in Nursing or Physician Assistant Studies Strong leadership, interpersonal, and problem-solving skills Excellent written and oral communication skills Punctuality, organizational skills, and ability to function within a team This role requires travel to and from one or more assigned facilities. Depending on travel amount, Curana may in the future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required. If so, you will be provided with thirty (30) days notice. Periodic motor vehicle (MVR) checks may be conducted as a condition of continued employment. Company Information
