Job Overview
This position involves patient follow-up and evaluating patient adherence to physician orders. The role includes patient education, assessments, documentation, and coordination of enteral care supplies and services in the patient’s home or relevant setting.
Job Responsibilities
Provides patient education as an integral part of patient care according to patient need and policy.
Performs complete and professional patient assessments in accordance with policies and procedures.
Maintains current knowledge of enteral home care techniques and relevant enteral therapy concepts.
Represents the company through professional appearance, patient care activities, and written reports.
Gathers necessary information from referral source to ensure insurance coverage of each enteral patient.
Follows up on necessary paperwork to ensure payment for services rendered.
Maintains patient records in a complete and organized manner in compliance with accreditation standards.
Drives to admit, deliver, and pick up enteral formula, feeding pumps, IV poles, and related feeding supplies to patients\' homes or places of business.
Provides patient care, treatment, and service in a continuous and coordinated manner in settings appropriate to meet patient needs.
Promotes safe, effective patient and organizational environments, as well as feeding equipment use.
Participates in ongoing education, including in-services, training, and other activities to maintain and improve individual competence.
