Overview Overnight Case Manager at RiseWell Community Services — New York, NY. Full-time, overnight shift (11pm to 7am). RiseWell Community Services is seeking a Full Time Overnight Case Manager to support the Governor’s SOS (Safe Options Support) team. The role provides comprehensive care to street-homeless or subway-dwelling individuals in Brooklyn and Queens. You will work as a member of the overnight multi-disciplinary team to assess health and social needs of the homeless population, with responsibilities during late evening to early morning hours (including weekends and weekdays). Primary duties include community outreach/canvassing on the streets and subways in Brooklyn/Queens, coordinating referrals with appropriate overnight facilities, accompanying clients, advocacy, and ensuring warm hand-offs and follow-ups with SOS day teams. Maintain collaborative relationships with community partners including the Department of Homeless Services, MTA, local law enforcement, community hospitals, and housing drop-in shelters. Responsibilities Assess health and social needs of the homeless population as part of the overnight multi-disciplinary team. Conduct community outreach and canvassing on streets and subways in Brooklyn and Queens. Coordinate referrals with appropriate overnight facilities and provide accompaniment and advocacy for clients. Ensure warm hand-offs and follow-ups with SOS day teams. Maintain collaborative relationships with community partners (DHS, MTA, local law enforcement, hospitals, outreach teams, shelters). Qualifications Minimum of 4 years of experience in case management. Bachelor’s degree in Human Services preferred with previous case management experience. Clean valid NY State Driver’s License. Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.
