Requirements:
- CA RN Licensure
- BLS (AHA)
- EPIC Experience
-2 Years Case Management Experience
- 1 year of experience with knowledge of post-acute levels such as Home Health, Hospice, Palliative Care, SNF, LTAC, B&C, Sub-Acute and Acute rehab
